Immunizations
- Provides immunizations for infants, school age children and adults.
- TB testing.
- Flu Shots
- School physicals/work physicals (by appointment/call for fees)

Communicable Disease
- Following IDPH guidelines, all re-portable diseases are investigated.

- Cases are monitored and specific diagnostic services are offered.
- Community education and surveillance provided
Lead Screening Program
- Lead testing provided on children ages 6 months through 6 years. Children with elevated blood levels are case managed by a Registered Nurse. Counseling, education and referral services are offered.
Breast and Cervical Cancer Program
Screening Services Provided/Covered:
- Clinical Breast Exam(CBE)
- Pelvic Exam(PE)
- Screening Mammogram
- PAP Test
Eligibility Requirements:
Age:
- 40-64 years old for CBE/Screening Mammogram
- 35-64 years old for PE/Pap test
No Health Insurance
Women below the age of 40 with symptoms(i.e. breast lump) and/or
Women below the age of 35 with abnormal pap need case by case approval
Genetics Program
- All abnormal newborn screening reported by IDPH is followed up for repeat testing, referral and education.
- All failed newborn hearing testing reported by IDPH is followed up for repeat testing, referral and education.
- All SIDS deaths reported by IDPH are followed.
- A genetic screening tool is used to assess indicators of genetic disease-education and referrals on case by case basis.
- Community education provided
School Based Health Centers
Located in two high school and one preschool, these clinics provide health care services and health education to students enrolled.
